At The Meridian, we provide specialized post-hospitalization therapy and step-down support for individuals transitioning out of inpatient psychiatric care, residential treatment, or intensive outpatient programs. Our therapists are experienced in working with clients who are navigating complex mental health presentations including suicidality, self-harm, severe depression, trauma, emotional dysregulation, and borderline personality disorder — and we understand the unique clinical needs of clients in the early stages of outpatient recovery.

The period immediately following psychiatric hospitalization or inpatient mental health treatment is among the most clinically significant — and most vulnerable — in a person's recovery. Research consistently shows that the weeks following discharge carry the highest risk for relapse, crisis recurrence, and readmission.
